Required Identification for your Appointment
If you are 18 years or older:
The state of Wisconsin requires proof of your age to receive any body piercing. A valid state or government issued photo ID (ie. Driver’s license, temps, ID card, or passport) are accepted.
I cannot legally pierce you if you do not bring a valid ID to your appointment.
If you are 16 or 17 years old:
The state of Wisconsin requires minors to be 16 years or older and provide proof of age to receive a body piercing (excluding ear lobes, nipples, genitals, and surface piercings). A birth parent or legal guardian needs to be present at the time of the piercing. BOTH the minor AND guardian need valid state or government issued photo IDs (ie. Driver’s license, temps, ID card, or passport). A birth certificate or guardianship paperwork to prove their relationship is also needed.
I cannot legally pierce you if you do not bring valid IDs AND the correct paperwork to your appointment.
If you are under 16 years old:
In the state of Wisconsin I cannot pierce anything other than ear lobes on anyone under the age of 16. I also choose not to pierce any children under the age of 10. To get ear lobes piercings, the birth parent or legal guardian needs to provide a state or government issued ID (ie. Driver’s license, temps, ID card, or passport) AND a birth certificate or guardianship paperwork to prove their relationship.
I cannot legally pierce you if you do not bring valid IDs and the correct paperwork to your appointment.
